Dordaviprone is a first-in-class small molecule imipridone. On August 6, 2025, dordaviprone was granted accelerated approval by the FDA.⁶ It is used to treat patients with diffuse midline glioma harboring an H3 K27M mutation.⁵ Dordaviprone was initially studied as an anticancer agent that induces TNF-Related Apoptosis-Inducing Ligand (TRAIL).³ Dordaviprone has several modes of action, including the activation of mitochondrial caseinolytic protease P (ClpP), antagonism of the dopamine D2 receptor,⁵ activation of Activating Transcription Factor 4 (ATF4), and induction of endoplasmic reticulum (ER) stress response.³
