Bepirovirsen is an HBV mRNA-directed antisense oligonucleotide indicated with nucleos(t)ide analog reverse transcriptase inhibitor therapy for chronic hepatitis B virus infection in adults without cirrhosis. A medication given by injection, together with other antiviral medicines, to treat long-term hepatitis B infection in adults without cirrhosis.
- Mechanism
- Curator reviewed · 4 references
Bepirovirsen is a gapmer antisense oligonucleotide with five 2′-O-(2-methoxyethyl)-modified ribonucleotide wings flanking a central 10-deoxynucleotide gap, with phosphorothioate linkages throughout. It is designed to bind a conserved 20-nucleotide site present in HBV mRNAs, including the pregenomic RNA, precore, preS1, preS2/S and X mRNAs, forming a DNA:RNA heteroduplex that recruits endogenous ribonuclease H1 (RNase H1). RNase H1 catalyzes irreversible hydrolysis of the viral RNA strand, which degrades HBV mRNA and reduces HBV DNA and viral proteins, including HBsAg. The binding site is present in transcripts derived from both cccDNA and integrated HBV DNA.
