Pemivibart is a monoclonal antibody targeting SARS-CoV-2 spike protein used for pre-exposure prophylaxis against COVID-19 in certain immunocompromised patients. It received emergency use authorization (EUA) from the FDA in March 2024 for pre-exposure prophylaxis (PrEP) of COVID-19 in certain high-risk patient populations.

SARS-CoV-2 entry into the host cell is mediated by the angiotensin-converting enzyme-II (ACE2) receptor, which binds to the receptor-binding domain (RBD) of the viral spike protein. Upon entry, SARS-CoV-2 replicates and causes viral pathogenicity. Pemivibart is a recombinant human monoclonal IgG1λ antibody that targets the SARS-CoV-2 spike protein receptor binding domain (RBD), thereby inhibiting virus attachment to the human ACE2 receptor on host cells.
