Linvoseltamab is a bispecific human IgG4 monoclonal antibody used for the treatment of multiple myeloma. By binding to both targets simultaneously, linvoseltamab redirects T cells toward myeloma cells, forming an immunological synapse that activates T-cell–mediated cytotoxicity.

Multiple myeloma is characterized by the malignant proliferation of plasma cells that express B-cell maturation antigen (BCMA), a transmembrane receptor involved in B-cell differentiation and survival. T cells, through their CD3 receptor complex, can be harnessed to target tumor cells when appropriately engaged.
Linvoseltamab is a bispecific monoclonal antibody that binds to BCMA on myeloma cells and CD3 on T cells, bringing the two cell types into close proximity. This interaction forms an immunological synapse that activates T cells, leading to directed cytotoxicity against BCMA-expressing plasma cells. Upon engagement, linvoseltamab induces T-cell activation, proliferation, and the secretion of pro-inflammatory cytokines such as interferon-γ (IFN-γ), tumor necrosis factor (TNF), and interleukin-2 (IL-2), which further enhance anti-tumor activity and contribute to tumor cell lysis.
